Divine Designs Salon & Spa - Group Reservation Guest Information & Payment Guarantee Form

Deposit and Payment requirements for Group (up to four persons), multiple services (three or more hours of service), salon & spa package, third-party payment or "surprise" reservations require a guarantee by a valid major credit card or with a gift card that has sufficient funds to cover the cost of the entire reservation plus a prepaid group reservation gratuity amount if that is included in your event agreement.  Unless other payment arrangements are agreed upon your credit card will be charged a deposit of 50% of the estimated cost of the reservation within 24 hours of making the reservation or completion of this form, whichever is the sooner, with the balance due prior to or upon checkout.  Cancellations will be accepted without any penalty up to seven (7) days prior to your reservation.  Cancellation requests made between seven (7) days and 48 hours before your scheduled reservation will incur a cancellation fee equal to the amount of the initial deposit.  Any cancellations made less than 48 hours before your scheduled reservation are not permitted and will incur a forfeiture of the entire cost of the reservation.  Changes to your Group or Multiple Service Reservation are permitted without penalty, subject to availability, up to 48 hours prior to your scheduled date and time.

 

LARGE-GROUP (five more persons) require a payment guaranteed by a valid major credit card or with a gift card or electronic gift certificate with sufficient funds to cover the cost of the entire reservation plus a prepaid group reservation gratuity amount if that is included in your event agreement.  Unless other payment arrangements are agreed the group guarantor, or each self-paying guest, will be charged a deposit of 50% of the estimated cost of the reservation within 24 hours of receipt of a reservation itinerary via email or completion of this form, whichever is the sooner, with the balance due 7 (seven) days before the scheduled reservation date. Cancellations are permitted without any penalty up to fourteen (14) days prior to the reservation. Cancellation requests made between fourteen (14) days and seven (7) days before the scheduled reservation will incur a cancellation fee equal to the amount of the initial deposit.  Cancellations made less than 7 (seven) days before your scheduled reservation are not permitted and will incur a forfeiture of the entire cost of the reservation(s) being cancelled. Changes to your schedule are permitted without penalty, subject to availability, up to 48 hours prior to your scheduled date.
